DirectedPath can be contacted via telephone or email for all referral enquiries. We endeavour to match appropriate accommodation that will meet the needs of our clients.  We conduct pre placement assessments, although we also accept social workers assessments following a referral.

 

There is an opportunity for prospective residents, their families/carers and social worker to visit DirectedPath prior to moving in.
